A prescribed form which contains the personal details of the deceased and is required for reporting the deceased estate to the offices of the Master of the High Court in order to obtain Letters of Executorship. It must be completed by the surviving spouse, or failing that, a relative or any person connected to the deceased who is likely to have correct knowledge of the deceased’s personal details.
